A Health Care Directive gives your loved ones and providers the guidance to make medical and health care decisions for you when you are unable to do so.
A Health Care Directive should be in writing and dated. It should also be signed, witnessed, and notarized. It is a good idea to give signed copies to your family, doctors, etc. Make sure that your doctor is willing to follow your wishes. You should name a trusted person as your agent (person must be at least 18 years of age) to make health care decisions for you, and make sure you have an alternate in place just in case the first person in unavailable. Typical issues addressed are what types of treatments you may or may not want, where you want to receive your care, your goals and values and ideas about things like artificial nutrition and hydration.
